This must be done at least 1 month prior to the assessment taking place .

Enter each candidate’s details
Enter the syllabus and level
Enter the date of the assessment
Complete a Reasonable Adjustments Form for any candidate who has special educational needs including dyslexia, disabilities or temporary injuries (not necessary for Entry Level Foundation exams which are purpose designed)
If there is more than 1 assessor indicate that the candidate is in Group 1, 2 or 3 etc.

If you prefer, fax the form to us on 01695 228 003 or return it by post.

Final paperwork will be emailed to you at least 2 weeks prior to the assessment taking place and will contain all your assessor details.
